Iphone dials wrong number??

I have searched the forum but with no success
My iphone connects perfectly every time BUT sometimes when I try and make a call from the call lists ( not contacts) it dials the next number up the list.
ie list
John Smith
Hugh Grant
So I dial Hugh Grant, the display says Im talking to Hugh Grant but actually its called John Smith.
Makes sense???
Any help, ? I have deleted the phone and added again but its the same, its not a 'hard' problem but intermittent but frustrating..
